Why Now

Remote Care as the Next Step for Independent Primary Care

Medicare is changing what it pays primary care for. Four shifts are under way at once, and each one lands in the same place: the weeks between a patient’s visits. For a practice whose Medicare patients carry hypertension, diabetes and kidney disease together, those weeks decide how the year goes.

02
Westview Family Medicine

A Practice Built Around Its Patients

For nearly twenty years Westview Family Medicine has cared for families in the West Valley and central Phoenix, in English, Vietnamese and Spanish. Seven clinicians see patients six days a week, with labs and diagnostics in the office. Its Medicare patients carry more chronic illness than the national average, about half of its traditional-Medicare patients also qualify for AHCCCS, and since 2023 the practice has taken part in an Enhanced-track Medicare Shared Savings Program ACO.

2.1The Patients Westview Serves

An older, multilingual Medicare panel in which hypertension, high cholesterol and diabetes are the norm and kidney disease, heart disease and memory loss are common.

What a Westview Medicare Panel Looks Like

Share of traditional-Medicare patients with each condition, from CMS CY2024 claims for Westview clinicians.
Hypertension

At least three in four patients

Hypertension sits at CMS’s 75% reporting cap for most Westview clinicians, so the true share is higher. Home readings catch what one office reading misses and let the care team adjust medication month to month instead of visit to visit.

Diabetes & Kidney Disease

About half with diabetes

46% of Westview’s traditional-Medicare patients have diabetes and 28% chronic kidney disease. Most carry two or more chronic conditions, which is the patient chronic care management was written for.

Memory & Mood

Care that already reaches past the visit

About 20% of the panel carries a dementia diagnosis and 16% depression. Westview already runs cognitive assessments and care plans for these patients; monthly care management keeps the plan moving between visits.

2.2A Practice Built for Growth

Each step below added offices, clinicians or capability. Click through the years. The next step adds the part none of them covers: daily readings and a care team between visits.

2.3Remote Care Closes the Loop

Each part of Westview touches a patient at one moment: an office visit, a wellness visit, a cognitive assessment, a lab draw, a hospital discharge. A remote care service line connects them, so a patient who leaves one visit is still being watched until the next one. Select any part of the practice to see what changes.

Tap a part of the practice on the ring
What the ring does not have yet. CY2024 Medicare claims show chronic care management billed for about 130 traditional-Medicare patients under one Westview physician, and no remote physiologic monitoring or transitional care management at meaningful scale by any Westview clinician. The ACO’s care-coordination staff support some of its assigned patients. Daily readings for the hypertension and diabetes panel, and monthly care management for every eligible patient of all seven clinicians, are open ground.

The Operating Model

Building a Practice-Wide, Closed-Loop Remote Care Service Line

Westview’s physicians and advanced practice clinicians set the protocols and own every clinical decision. CoachCare staffs and runs the program. Everything runs through the eClinicalWorks system Westview already uses, from the first flag on a patient’s chart to the claim at month end.

APanel Risk Stratification & Patient Identification

The program starts with the panel, not a referral pad. eClinicalWorks already holds the diagnoses, vitals, labs and medications. Patients with uncontrolled hypertension, diabetes, kidney disease or several chronic conditions are flagged using criteria Westview’s clinicians set, so the right patients surface at a visit, at an annual wellness visit, or on an outreach list.

Emergency
Straight to 911

Chest pain, new shortness of breath or stroke signs reported on a call: the care team calls 911 with the patient on the line, then notifies the office. A critical reading on its own goes to the office. This cannot be configured away.

CMonthly Reporting & Revenue Cycle

At month end, three things come out of the program: clean claims in eClinicalWorks, an operational report for the practice’s leadership, and a clinical outcomes report for the clinicians.

Revenue cycle

Automated Claim Creation in eClinicalWorks

CoachCare is the only care-management application integrated with eClinicalWorks that creates claims automatically. No one builds a claim by hand, patient by patient, month by month.

  1. Time and readings captured as the care team works, against each patient’s care plan.
  2. Claim built in eClinicalWorks when a code’s requirements are met: 16 reading days, 20 minutes, two conditions.
  3. Westview’s billing team reconciles the batch in its own work queue.
  4. Submitted under Westview’s billing entity and NPIs.

Value Analysis

Clinical, Financial and Operational Value

A 24-month forecast for a Westview remote care service line: remote monitoring and chronic care management across the Medicare panel, priced at Arizona’s CY2026 Medicare rates with every patient priced at the Medicare amount, alongside what the program does for patients and for the people who run it.

Clinical Value

Clinical 1

Better connectivity, better care

Four office visits a year leave 361 days with no data. RPM bills only when a patient takes readings on at least 16 days in a month, so an enrolled patient gives the care team at least 192 days of readings a year.

Clinical 2

Blood pressure control you can document

Hypertension is the most common condition on Westview’s panel and the one home monitoring moves fastest. Home averages give the care team a monthly reason to adjust medication, and the documented result is the home number, not one rushed office reading.

Clinical 3

Better quality scores

Westview’s clinicians take part in an Enhanced-track Medicare Shared Savings Program ACO, which is scored on a fixed set of quality measures. Most of them depend on work this program does every month. Select a measure to see how.

Home BP averages give an accurate reading of control and a monthly chance to act on it. The patient’s documented status in eClinicalWorks reflects home data, not a single rushed reading in clinic.
Monthly chronic care management keeps A1c testing and medication changes on schedule, and home glucose readings show the care team where control is slipping before the next lab.
CMS calculates this from claims: unplanned admissions among patients who carry several chronic conditions. Daily readings, outreach on a confirmed trend, and a fixed cadence after any emergency visit or admission are how a program moves it.
PHQ-2 and PHQ-9 screening fits into monthly care management, and a positive screen is routed for follow-up. Depression is common in chronic disease and drives poor adherence.
A transitional-care call within two business days, devices home before the follow-up visit, and readings from day two are the post-discharge cadence that keeps a patient out of the readmission count.
Clinical 4

The right patient in the right appointment

Monitoring the panel every day tells the practice who needs to be seen now, not only who is next on the schedule. Patients surface from their home data, not only when they come in sicker.

Clinical 5

Post-discharge monitoring

When a Westview patient goes home from the hospital, the next 30 days decide whether they come back. Transitional care management pays for that month; remote monitoring and chronic care management carry the patient past it.

Only one device code (99454 or 99445), one first-tier management code (99457 or 99470) and one care-management program (CCM or APCM) bill per patient per month. About 51% of Maricopa County’s Medicare beneficiaries are in Medicare Advantage. Those plans must pay at no less than the Medicare rate, a floor; individual contracts set their own terms for the care-management code families, and the forecast prices every patient at the Medicare amount.

Financial 2

Financial Value in Either Deployment

Westview can hand CoachCare the remote monitoring and keep chronic care management with its own team, or run both programs through CoachCare. Either way the program is net-positive. Select a deployment to redraw the charts.

Active Program Enrollments

Monthly census by program. A patient on both RPM and CCM counts once in each. Both programs reach the eligible share of the panel inside the first year (remote monitoring by month 5, chronic care management by month 6), so the size of Westview’s Medicare panel, not enrollment capacity, sets the ceiling. A larger chart count moves this forecast almost one for one.

Why CoachCare for Westview Family Medicine

Why Work with CoachCare?

Six reasons this partnership fits Westview specifically, not remote care in general.

It builds on what Westview already does

Westview already bills chronic care management, annual wellness visits and cognitive care planning. Remote monitoring adds daily readings to that work, and CoachCare can take on as much or as little of the care management as Westview wants.

It runs inside the chart you already use

Built on eClinicalWorks’ own workflows. Readings, documentation and claims live in the chart, and enrollment status is visible at every office.

Fully staffed, no hiring

CoachCare’s care managers carry the caseload at 165 patients each. Enrollment, devices, business-hours alert triage and billing preparation are on CoachCare’s payroll, and fees are per active patient per month, with no capital outlay.

Clinicians stay in charge

Westview’s physicians and advanced practice clinicians set the protocols, sign the care plans and make every clinical decision. Claims go out under Westview’s own billing entity and NPIs.

Designed for Westview’s patients

Cellular devices that need no home internet and nothing to pair, and enrollment, device instructions and outreach scoped with Westview for patients who prefer Vietnamese or Spanish before the first patient enrolls.
